Your Rights Against Surprise Bills
Under the federal No Surprises Act, you have the right to receive a Good Faith Estimate of expected costs before scheduling services.
YOUR RIGHTS AND PROTECTIONS AGAINST SURPRISE MEDICAL BILLS
When you get health care services at Nalu Wellness, we are required by law to give you this notice about your billing rights.
Right to a Good Faith Estimate
You have the right to receive a Good Faith Estimate explaining how much your care will cost before you schedule a service.
Under the law, health care providers need to give patients who don't have insurance or who are not using insurance an estimate of the expected charges for health care items and services.
- You have the right to receive a Good Faith Estimate for the total expected cost of any health care items or services
- Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your scheduled service
- You can also ask your health care provider for a Good Faith Estimate before you schedule an item or service
- If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill
- Make sure to save a copy of your Good Faith Estimate

Insurance-Billed Services
If you are using health insurance (Aetna, Kaiser, Heraya Health, or auto injury coverage), your actual out-of-pocket cost will depend on your specific plan, deductible, copay, and coverage limits. We strongly encourage you to contact your insurance carrier before your first appointment to understand your benefits and expected cost-sharing.
We will provide you with a Good Faith Estimate of our billed charges and any known cost-sharing amounts before your scheduled appointment upon request.
